Terms of Service (TOS)

1. Acceptance of Terms

By booking an appointment or using Sunnyvale Concierge Medical Group services, you agree to these Terms of Service. If you do not agree, please refrain from using our services.

2. Services Provided

Sunnyvale Concierge Medical Group provides multi-specialty concierge medicine, telemedicine, case management, care coordination, behavioral health, physical/occupational therapy, and access to U.S.-approved medications.

3. Appointment & Payments
  • Patients are responsible for ensuring accurate personal and medical information is provided.

  • Payments for services and medications must be made directly to our approved partners (e.g., pharmacies, hospitals) where applicable.

  • Missed appointments without notice may be subject to rescheduling fees.

4. Referrals & Coordination

We coordinate patient referrals both within Nigeria and the United States. While we vet all referral facilities, final responsibility for care delivery rests with the receiving provider.

5. Telemedicine Limitations
  • Telemedicine requires stable internet access.

  • Patients may choose in-clinic telemedicine support if home connection is unstable.

  • Not all conditions are appropriate for telemedicine; in such cases, in-person care may be required.

6. Patient Responsibilities

Patients agree to:

  • Provide accurate medical history.

  • Follow treatment recommendations.

  • Use medications as prescribed.

7. Limitation of Liability

Sunnyvale Concierge Medical Group is not liable for delays, technical failures, or outcomes outside of our direct control, including third-party facilities and pharmacies.

8. Amendments

We reserve the right to update these Terms at any time. Updated terms will be posted on our website.
